A severe health issue is rapidly gaining momentum across the United States, with many individuals unaware of its presence. Known as Metabolically–dysfunction–associated steatotic liver disease (MASLD), it ranks among the most prevalent liver diseases nationwide, already impacting millions. The alarming factor is its silent progression, typically revealing itself only after the liver sustains serious injury.
MASLD emerges when excessive fat accumulates in the liver, frequently affecting individuals with existing metabolic disorders such as obesity and type 2 diabetes, according to Dr. Leana Wen, a noted medical professional. Distancing itself from other liver complications, MASLD is not related to excessive alcohol consumption. Formerly termed nonalcoholic fatty liver disease, it was renamed to highlight its close association with metabolic health.
Experts are increasingly recognizing MASLD as one of the most widespread chronic liver diseases globally. Research indicates that between 30% and 40% of adults worldwide might be impacted. In the U.S., approximately one in three adults is affected, with the prevalence spiking among those who are overweight or have diabetes — in these demographics, up to 70% to 80% could be living with MASLD. Beyond liver complications, MASLD poses risks for heart disease, strokes, and certain cancers. When neglected, it can lead to inflammation, scarring, cirrhosis, liver failure, or even cancer over time.
A critical concern is that many individuals with MASLD do not experience symptoms in the initial stages. Often, there are no obvious indicators. When symptoms do manifest, they tend to be mild, like fatigue or minor stomach pain, and can be easily dismissed. As liver damage progresses, more severe symptoms may arise, including jaundice, dark urine, swelling, confusion, or extreme fatigue—often indicating substantial damage has occurred.
Diagnosis of MASLD frequently occurs inadvertently during routine medical evaluations. Blood tests may reveal elevated liver enzyme levels, or ultrasounds could detect fat within the liver. Upon identifying potential MASLD cases, doctors tend to assess other risk factors such as weight, blood pressure, blood glucose levels, and cholesterol. Consequently, health experts advocate for early screening, primarily for individuals who are overweight or diabetic, since most patients lack early indications and may stay unaware for extended periods.
Individuals with metabolic issues carry the highest risk. Notably, MASLD is not exclusive to adults; it's increasingly recognized in children. Nearly 10% of Americans aged 2 to 19 might already be affected. The highest incidence of cases in the U.S. is reported among Hispanic individuals, followed by non-Hispanic Whites and Asian Americans. Lifestyle choices, dietary habits, and metabolic health significantly influence disease development.
On a positive note, early intervention can often reverse MASLD. Healthcare professionals assert that weight loss remains the most effective treatment, significantly reducing liver fat and inflammation. Regular physical activity plays a supportive role as well. New treatment options, including Rezdiffra and Wegovy, have been introduced for advanced MASLD stages, yet lifestyle modifications are deemed vital for most patients.
To safeguard long-term liver health, health experts encourage maintaining a healthy weight, engaging in physical activities, and consuming a balanced diet rich in whole grains, vegetables, and lean proteins. Additionally, they advise reducing sugary and processed foods, managing conditions like high blood pressure, high cholesterol, and diabetes. Periodic discussions about liver health during medical check-ups are paramount, with simple tests available to detect early issues before serious harm ensues—a hepatitis vaccination can also enhance liver protection.
As MASLD prevalence escalates across the United States, medical professionals are stressing the importance of proactive measures. While this condition can remain dormant for years, early recognition, establishment of healthy habits, and regular check-ups can play a crucial role in safeguarding liver health and preventing irreversible damage.
