An inmate has passed away while in custody at Millhaven Institution, as confirmed in an official announcement by Correctional Service Canada on April 8, 2026. This incident took place at the facility's Regional Treatment Centre designed for specialized inmate care.
The deceased has been identified as Kenneth Irwin, 69 years old. He died on April 7, 2026, with preliminary reports indicating the cause of death was natural. At the time of his death, he was serving a three-year and six-month sentence, initiated on August 8, 2025.
Correctional officials indicated that appropriate procedures were executed post-incident. The next of kin has been informed, and measures are in place to facilitate transparency and to reassure that the situation has been handled properly. Currently, no suspicious circumstances have been identified.
In alignment with standard operating procedures, a thorough internal review will be conducted regarding the circumstances of the death to verify adherence to all policies and to discover any potential areas for enhancement.
Furthermore, both the police and the coroner have been notified, as mandated by federal correctional guidelines, to assure an independent evaluation of the situation and corroborate the cause of death.
Officials reiterated that the health, safety, and well-being of inmates are paramount within the Canadian correctional system. Regular assessments are conducted in all cases of inmate mortality to ensure accountability and to uphold institutional integrity.
This case underscores the systematic measures taken by correctional bodies in Canada when an inmate dies while incarcerated, maintaining a focus on care, transparency, and oversight.
