A video showing a cycle ambulance operating in Karachi has gone viral across social media, triggering widespread debate and online criticism. Many users mocked the pedal-powered emergency vehicle, comparing it with advanced medical transport systems such as air ambulances, flying intensive care units (ICUs), and drone-based emergency services available in several countries.
The viral video led many social media users to believe that Karachi Mayor Barrister Murtaza Wahab had recently introduced the initiative. The claim quickly spread online, resulting in sarcastic comments and memes questioning Pakistan's pace of technological advancement.
One viral post joked that while the rest of the world was developing flying ICUs, Karachi had introduced a cycle ambulance. Another user sarcastically wrote that Pakistan had "shocked the world" with its latest invention and that humanity would always remember its contribution.
Contrary to the viral claims, the cycle ambulance programme is not a recent launch. Local media reports confirm that the service has been operational since May 2025 and was introduced as part of a pilot project by the Sindh Integrated Emergency and Health Services (SIEHS-1122), also known as Rescue 1122.
The initiative was created to improve emergency medical response in Karachi's densely populated localities, where traffic congestion and narrow streets often prevent conventional ambulances from reaching patients on time.
The cycle ambulances were specifically designed to overcome one of Karachi's biggest challenges—limited access to patients in heavily congested neighbourhoods.
Unlike standard ambulances, the specially equipped mountain bicycles can quickly navigate through traffic jams, narrow lanes and crowded residential areas. Their primary purpose is to ensure that trained emergency responders reach patients as quickly as possible and provide immediate medical assistance before a conventional ambulance arrives.
This first-response model aims to reduce delays during medical emergencies, where every minute can make a significant difference.

The programme was introduced not only to strengthen Karachi's emergency healthcare network but also to encourage greater participation of women in community rescue and emergency response services.
Officials behind the initiative said the project reflects an effort to improve healthcare accessibility while promoting women's involvement in public emergency services.
Although the video attracted significant online ridicule, many users defended the initiative, arguing that it serves a practical purpose rather than being a symbol of technological backwardness.
Supporters pointed out that cycle ambulances are a cost-effective and efficient first-response solution for congested urban areas where larger emergency vehicles cannot reach patients quickly. They argued that the programme should be evaluated based on its ability to save lives rather than its appearance.
The Karachi cycle ambulance has resurfaced online because many users mistakenly believed it was a newly launched project. However, official reports indicate that the initiative has been functioning since 2025 as a pilot emergency response programme.
